Improvements

• DEM-51 - Include a standard version End Point that includes the current version number (populated by a token) in the description
• DEM-52 - Update getDigestedHistoryData to accept an end date. If no end date is supplied, default to today
• DEM-59 - Update the exporter to cope with Case Management converted cases. Now all Case Management cases are exported using the latest value of their caseType rather than history labela (which is immutable). Note there is no visible change in the Data Exporter interface, the change is completely internal
• DEM-65 - Now that the exporter can cope with converted cases, make sure the exports for "notes" and "null" labelc also export the correct histories
