Build a One Page Form
Create a new form, ensure the Form Name is named correctly (no spaces or special characters and it cannot start with a number) and Description fields.
Add a 'Vertical Layout' ensuring that you have named it and provided a label that is clear for the customer. The Label on your first layout is the heading for the page, if you have got 1 question on your page then the Label should be the question, but if you have multiple fields on the page then your Label should be a description of the page for example 'Contact us', 'Your information', 'Feedback' etc.
Inside the Layout add:
Text Input field - Name = NAME, Labelled 'Name', Error Message = Please provide your name
Text Input Field - Name = EMAIL, Label = Email Address, Pattern = Email Address, Error Message = Please provide a valid email address
Textarea input basic - Name = FEEDBACK, Label = Feedback, Required = False
Submit Button - Name = SUBMIT
Save to Database Action - Name = SAVETODB
Confirmation message action - Name = THANKYOU, Message = Thank you for filling in the form, we will review and get back to you within 3-5 working days
Under Form settings scroll to the bottom and set Field Label Marker to Mark Optional Fields
