About this Form

This form lets a user report a single missed bin or an entire street's worth.

Details

This form uses our standard snippet to capture a user's name and address, then lets them select the bin type that was missed form a drop-down field. It provides a great starting point for a full missed-bin reporting process.

Output

The form has placeholder action fields to send and email, start a workflow process and display a confirmation message to the user. You can update or remove these depending on how you'd like to use the form.
