A simple standalone form to offer support during the COVID-19 pandemic. The form asks for the details of the type of support and some details to contact them.
Be aware that this is a real form provided by the Gateshead web team so it has some specific elements to meet their use case. They will probably not be necessary for your use case but are left in place in case they can be easily modified to meet your own and are a good example. In this case the form checks to ensure it is within a ward in Gateshead and tags the request so it can be easily routed to the most appropriate case manager.

Output
The contact details are saved to the digital platform (Please ensure you have a process to clear them down once processed). Two emails are then generated, 1 back to the volunteer and another to the staff members dealing with the organising of support.
