The form to book a death registration appointment checks that the user has the correct information to be able to register the death, takes their details, then lets them pick an available time slot.
Location and Certificates
The first page of the form checks whether or not the death occurred in the local authority area. If it didn't the user is able to make a registration by declaration.
This page also checks that the user has the appropriate certificate. If not the user may still be able to book the appointment if advised to do so by a coroner.
Attendance and Details
This page collects details about the person making the appointment and the deceased. This includes the relationship of the person to the deceased, their name and contact details, the name and date of death of the deceased and whether or not the person attending the appointment is bringing an interpreter with them. If using an interpreter extra time, set in the configuration options, will automatically be added to the appointment.

Selecting an Appointment
Based upon the configured length of time for an appointment and interpreter all users are given the duration of their appointment on this page.
Appointments booked by staff users via the slot booking calendar display the selected calendar, date and time slot at the top of this page. Beneath these details is a list of the next immediately available appointments.
Members of the public using the booking forms will see a list of the next five available appointments, with the option to show more dates. Selecting another date loads all of the available time slots on that day. Logic in the booking form checks that the appointment is within five days of the supplied date of death and displays a message if it is not.
Check Your Answers
The final page of the form lets the user check all of their answers and jump back to any page that needs editing.
When this page is submitted the appointment is booked and notification emails are sent.
Customisation
The text of the booking form is set using the Template Manager. In the Registrars application you'll find templates for each of the appointment types. For death registration the following can be set.
