Details

This form is designed for people waiting for a care assessment by the Adult Social Care team, or their care assessment is complete and they are eligible for support.

It asks for individual circumstances to work out if the user can contribute to the cost of council-funded care, and how much that contribution would be.

Output

The form is designed to start a Case Management case called "financialassessmentrequest". This can be modified on the final page of the form.
