Use this application form to apply for School Travel if:

• Your child has Special Educational Needs and Disabilities (SEND)
• Your child's case has been discussed at an In-Year Fair Access Panel Meeting (IYFAP)

Output

The form is designed to start a Case Management case called "ptsendapplication". This can be modified on the final page of the form.
